Our usual office hours are Monday through Friday 9:00 to 3:00.

Our office is at 109 Ponemah Rd. (Rte 122) Spruce Park Professional Center, Office #5, Amherst NH 03031.

• Please complete your FAA 8500-8 "Application for Airman Medical Certificate" online before your appointment.
  Paper forms are no longer accepted by the FAA.
• Please bring the confirmation number to your appointment, we cannot proceed without it.
